โจ Understanding these states—like crying, alertness, and sleep—can help caregivers better support their baby’s needs and responses.
Many people think a baby doesn’t show signs of hunger until they’re crying—but by that point, they’re often too dysregulated to feed successfully. ๐ฅ Crying is a late hunger cue, and in this state, babies may struggle to coordinate their breathing and motor plans for feeding.
๐ Quiet Alert is the optimal state for feeding. Babies are most organized and regulated in this state, making it easier for them to feed effectively while maintaining coordinated breathing and motor movements. Recognizing early hunger cues and aiming to feed during the quiet alert phase can reduce frustration for both caregivers and babies.
๐ Crying: A baby’s signal of discomfort or unmet needs
๐ Active Alert: High energy, often ready for interaction but vulnerable for becoming fussy and transitioning to crying if their immediate needs are not met
๐ Quiet Alert: The sweet spot for bonding and feeding

Building a connection during feeding time lays the foundation for a strong attachment and sense of security between an infant and their caregiver. When caregivers are present, attentive, and responsive to their infant’s communication cues—such as rooting, sucking, or crying—the infant learns that their needs will be met with care and consistency. This interaction not only fosters trust but also helps the infant develop interoception, the ability to recognize and understand their body’s internal cues, such as hunger and fullness. Over time, this awareness becomes a critical skill, supporting the development of self-regulation, social-emotional understanding, and positive mealtime experiences as the child grows. ๐ชด

๐What you don’t see: The mom of two, one of whom is neurodiverse with ADHD and Anxiety, who often struggles at school, in social environments, and in new environments.
My daughter’s world often includes flappy happy hands, anxious finger picking, difficulty with transitions, self-doubt, hyperfixations, emotional dysregulation, social anxiety, language shut-downs, and shame puddles that leave us both in tears.
My daughter’s world also includes a deeply caring heart, loyalty that goes on forever, a creative brain that is constantly coming up with new ideas, a huge smile, the best hugs, energy that I want to bottle up and sell, and passion that will set her apart from others as she grows.
When I talk about connecting with a child—meeting them where they are, embracing their meaningful activities, and leaning into their individual differences—it doesn’t just come from being a speech-language pathologist. It comes from being a mom who fiercely advocates for my child, has cried through many hard days, and fought to create a space where she feels loved for exactly who she is.

Did you know that for individuals living with Pediatric Feeding Disorder (PFD) or Avoidant/Restrictive Food Intake Disorder (ARFID), trying a new or non-preferred food can feel overwhelming? Taking a single bite might be miles outside their comfort zone. That’s why I’m participating in the #OneBiteChallenge—to grow understanding and awareness of what these challenges look and feel like at every meal.
For some, starting with a bite is too much. Progress might look like simply tolerating the food on their plate, smelling it, or giving it a quick lick. Every small step is a victory! ๐ช
This challenge isn’t about endorsing a specific treatment—it’s about fostering compassion and shining a light on the lived experience of those navigating PFD and ARFID daily. I encourage those who feel comfortable to participate in their own way, using a food they select with safety and allergens in mind.

The key is applying the right amount of challenge to help your child stay engaged and grow. Finding the just right amount of tension is where learning happens, but it can feel really scary to explore that tension.

Pediatric Feeding Disorder (PFD) affects up to 76% of children with autism, highlighting the importance of a neurodiversity-affirming approach in feeding therapy. ๐ง
โจ Feeding therapy isn’t just about introducing food—it’s about ensuring that a child’s nervous system can process sensory information and autonomic responses.
Before food ever comes to the table, my role is to help the child feel regulated, safe, and connected. That may mean working through some challenging moments of dysregulation, but it is something that the child and I work through together ๐ซถ๐ป and how we strengthen our connection.
๐ Key areas like sensory regulation, motor planning, neuroception, and interoception must be addressed first. By incorporating play that meets a child’s unique needs, I work to create opportunities for engagement and learning.

Just like flowers need care and nurturing, so do our feeding moments with little ones! ๐ธ A connected feeding isn’t just about nourishing their body; it’s about creating an experience that fills their heart too. Here are 7 key ingredients for a connected mealtime:
1๏ธโฃ Being available to receive your infant’s feeding cues. Babies aren’t able to tell us with their words yet what they need, how much they need, and if they’re feeling overwhelmed. Check in with yourself during feeding times and identify if you were feeling common present, distracted, or overwhelmed.
2๏ธโฃ Honoring your infant’s cues. It’s not just about receiving your infants cues, but also honoring those cues and trusting that they already understand what their body
3๏ธโฃ Waiting until baby is hungry. It seems simple that we want our infant to be hungry and ready to eat, but sometimes we feel driven to adhere to outside schedules and not following our infant’s hunger cycle.
4๏ธโฃ Support for baby’s body. When baby’s body is not aligned, it makes it harder for their feeding muscles to coordinate. Check in and see if you could draw a straight line from their nose to their belly button (if you’re bottle feeding make sure the bottle is in this pathway too!)- or if they are on their side, check if you could draw a line from their ear, shoulder, and down to their hip.
5๏ธโฃ Mind the flow. Not all “slow flow” or “level 1” nipples release the same volume of liquid per minute. If you see liquid spilling, hear gulping, or baby looks stressed- consider trying a slower flow
6๏ธโฃ Positioning matters! Just like the nipple flow rate, some feeding positions make liquid flow faster and can overwhelm baby. Raising their head up or turning them into their side can help baby feel more in control.
7๏ธโฃ Not all nipple shapes are created equal either! A cylindrical, elongated teets allow for tongue cupping and eliminate excess air from being swallow because it completely fills their oral cavity.

We often think eating is a simple 2-step process: grab the food, then eat it. But in reality, eating is a complex experience involving the entire body working as one. ๐ง ๐ฆด
It takes 26 muscles and 6 cranial nerves working together to safely move food and liquid through the body. It also engages all parts of the sensory system, including interoception (the sense of what’s going on inside our bodies).
For kids, exploring new foods can feel overwhelming—it’s not a one-size-fits-all process. ๐ฅ A child may need to approach different foods at different levels, and those levels can shift day-to-day or meal-to-meal based on their interest and how safe their body feels.
โจ As adults, we can relate when we think about our own “red zone foods” (the ones we avoid) or “yellow zone foods” (the ones we’ll eat, but they’re not our favorites).
๐ฌ What’s a yellow zone food for you? And what’s your “just-right” way of eating it—do you need it prepared a certain way, do you need to mask the flavor with a dip, do you need to chase it down with a different flavor? Drop your thoughts in the comments below!

Bottle-feeding not going as smoothly as you’d like? Here are 3 simple strategies to try:
1๏ธโฃ Change the position: Sometimes just lifting your baby’s head a little higher can slow the flow and make feeding more comfortable. ๐
2๏ธโฃ Add pacing support: Fill the bottle nipple halfway to give your baby more control over the flow. This helps them keep up and feel more in charge of the feeding. ๐ผ
3๏ธโฃ Slow the flow: Swap to a lower-level nipple to reduce the speed without needing a whole new bottle system. ๐ฏ

โ๏ธ and here’s why: Oral motor tools aren’t task-specific, meaning they don’t connect to real, functional tasks like eating. Food isn’t plastic or silicone, and it certainly doesn’t vibrate! These tools don’t prepare kids for when food actually breaks off during mealtimes. ๐ฝ๏ธ
I’ve also worked with too many kids who had these tools put in their mouths against their will, only to develop stronger oral aversions. ๐ Let’s work with food first, let kids stay in control of their bodies, and keep oral experiences positive—always! ๐

There are a number of things that can mimic the signs of a tongue tie during feeding. Here are three common ones:
โจClicking sounds: While clicking can be a sign of a tongue tie, it can also indicate that the milk flow is too fast for baby to keep up with, causing the clicking sound as they try to manage the flow.
โจShallow latch: A shallow latch could point to a tongue tie, but it can also happen when breasts are engorged or too firm, making it harder for baby to latch deeply.
โจPulling away from feeding: Babies with tongue tie may struggle to feed and pull away, but this could also be due to discomfort from medical conditions like reflux.

Building confidence with food starts with exploration! Here are 5 playful ways to turn mealtimes into mini adventures for your worried or skeptical eater:
1๏ธโฃ Count the pieces! Whether it’s blueberries or crackers, counting how many pieces you can find keeps little hands busy and encourages interaction.
2๏ธโฃ Find the rainbow! Can you spot all the colors in your meal? Searching for reds, greens, yellows, and more can make new foods less intimidating. ๐
3๏ธโฃ Smash it! Give them permission to squish! Smashing foods like bananas or peas helps ease anxiety around textures.
4๏ธโฃ Makeup time! Pretend the food is makeup. Swipe some mashed potatoes as “blush” or yogurt as “lipstick”—it’s all about fun! ๐
5๏ธโฃ Bring toys to the table! Invite action figures or favorite dolls to “try” the food first. Let toys lead the way in exploring the meal!

What does this have to do with feeding? ๐ค
When it comes to mealtimes with our eaters, it’s all about fostering a relationship built on safety, trust, and honoring the child’s autonomy. Your child should feel safe enough to explore food at their own pace—and equally safe to reject it. They need to trust that you, as their caregiver, will respect both their engagement and their choices to not engage. ๐ชด
The key is to “entice, not intrude”. Allow your child to explore and learn about food naturally, without feeling pressure. When we, as caregivers, step in and intrude on their exploration, that’s when feelings of pressure arise, and the safety and trust we’ve worked to build can slip away. ๐
Next mealtime, pause and ask yourself: Am I enticing my child to explore, or am I unintentionally intruding? ๐ฑ

Yesterday we talked about how to know when your baby is ready to eat…So today we are going to talk about the other end of the spectrum- “How do I know if my baby is NOT ready to eat?”
๐ค It can be tricky, but recognizing the signs of overwhelm or disinterest is just as important as knowing when they’re hungry. Here are some cues that your baby may not be ready for feeding or feeling overwhelmed:
โ Asleep or drowsy (we want babies awake and engaged during feedings!)
โ Not opening their mouth when the nipple is offered
โ Pushing the nipple out or turning their head away
โ Arching their back, pushing the nipple away
โ Gagging, coughing, or not sucking
โ Falling asleep quickly but waking up when the nipple is removed
โ Hiccups, yawning, sneezing, gulping, heavy breathing
โ Furrowed brow or wide eyes
For babies starting solids, these cues are often the same. It’s important to watch for signs of disinterest or overwhelm and respond accordingly. Following your baby’s cues for hunger and fullness is key to fostering a strong bond, developing secure attachments, and creating positive feeding experiences. It also helps to promote healthy growth and development! ๐ช๐

“How do I know if my baby is ready to eat?” ๐ค It’s not always as simple as it seems! Babies, especially those younger than 3 months, communicate hunger in unique ways. Here’s what to look for:
โ๏ธ Awake and alert
โ๏ธ Hands and arms toward midline (no floppy babies)
โ๏ธ Licking, lip smacking, rooting
โ๏ธ Sucking on fingers
โ๏ธ Good coloration and moving toward the feeder
For infants starting solids, signs of readiness are different. Look for:
โ๏ธ Stable, independent sitting
โ๏ธ Reaching for solids and showing interest in others eating
โ๏ธ Opening their mouth as food approaches
At this stage, babies may begin self-feeding but can also be fed by a caregiver who is responsive to their hunger and fullness cues. Following your baby’s cues for both breastfeeding/bottle-feeding and solids is essential for building a strong bond, secure attachment, and positive feeding experiences. It also helps to optimize growth and healthy development! ๐ช๐

As caregivers, we play such an important role in nurturing a safe, trusting, and connected mealtime relationship. By reminding our worried eaters that they are in control of their bodies, that their safe foods can remain safe, and that ‘no’ is a full sentence, we create an environment where they can feel secure. Mealtimes should never be a battle, but a space where our kids know they are respected, even if they choose not to eat. Food exploration doesn’t always mean tasting—it can be smelling, touching, or just looking. ๐
And please, please, PLEASE let your child’s safe foods stay safe. This may mean advocating for your child and speaking up to therapists when they try to modify those foods in therapy. (I’ve had families tell me whacky stories about their therapist having their child dip pirate’s booty into strawberry yogurt and ice cream ๐คช). Messing with safe foods runs the risk of your child losing that food because it’s no longer viewed as safe.
When we integrate these values into our mealtime routine, we empower our children to trust their bodies and feel connected, not pressured. Let’s turn mealtimes into moments of growth, not stress. ๐

When my daughter is so dysregulated that even brushing her teeth or getting dressed feels like an impossible task, I get it. I feel that overwhelming frustration when I can’t just ‘make’ things happen, no matter how much I need them to. I’ve cried in the car because it feels like too much.

Truth? It’s actually VERY normal for infants to spit up and experience reflux.
โก๏ธApproximately 70-85% of infants experience spitting up within the first 2 months of life. These same infants experience resolution of symptoms by 12 months of age in 95% of instances.
๐ค The cause? An under developed mechanism called the lower esophageal sphincter. Its job is to hold tight so that the contents below don’t progress up into the esophagus. Over time and as an infant develops, the LES get stronger and better at keeping things down that are supposed to stay down.
๐ก When should you be concerned? When the reflux occurs with coughing, gagging, choking, forceful vomiting, or is so uncomfortable that it interferes with sleep and/or activity!
๐คฉ It may be that the nipple flow rate needs to be slowed, the infant’s position needs to be adjusted, and/or pacing strategies need to be integrated into feeds. These less invasive treatment strategies should be explored before meds, thickeners, and tongue tie revisions.

I need your help deciding if I should change how often I post about specific feeding and swallowing topics. Currently, my goal is to talk about a specific feeding topic (infants, tube feeding) for a week at a time and rotate through topics weekly. I’m wondering if you would like to see me rotate through topics more frequently so that there is more variety. Tell me what you want to see!
๐ฉบBreastfeeding provides essential health benefits, like reducing the risk of SIDD, infections, and chronic conditions such as asthma and obesity. For Black babies, these benefits are even more crucial given the risks they face.
๐ก But here’s the thing- Black mothers don’t have the same access to lactation support, in general, let alone access to lactation support from providers that look like that. This disparity makes it that much harder to start and continue breastfeeding. By advocating for better access to culturally competent lactation services, we can help Black women give their babies the best possible start to life.
Another unbelievable statistic- Black infants cared for by Black physicians have a 39% lower mortality rate than those cared for by White physicians. ๐คฏ
Why am I talking about this issue? Because I know that I can use my platform as a white healthcare professional to advocate for my Black patients, be aware of my own bias’ and provide optimal care, and educate others about how important this topic is.
